To participate in the camp, it is REQUIRED that each camper submit the
Medical Information Form before the camp begins. Please Fax,

Every camper must submit the Medical Wiver. The Travel Consent form must be signed by a parent or guardian only if the camper is
under 19 years of age and will be picked up from the camp by a non-parent or
guardian, or the camper is traveling on their own to the camp. We must receive this
form prior to the camp’s start date to participate.
